Job summary

Tender Care Home Health & Hospice in El Paso, TX seeks a PRN Licensed Master Social Worker to collaborate with physicians and the home health team to address social and emotional factors affecting patient health.

You will conduct psychosocial assessments, coordinate care, assist with discharge planning, and connect patients and families with community resources. Prior healthcare social work and CPR preferred.

Qualifications

• Master’s Degree in Social Work and active state license.
• Minimum one year of social work experience in a healthcare setting.
• CPR certification preferred.

Responsibilities

• Assess psychosocial factors affecting patient care and plan.
• Assist physicians and team to coordinate care and discharge planning.
• Provide information, referrals, and resources to patients and families.
• Maintain patient records with timely documentation (within 48 hours).
• Act as liaison with community resources and agency staff.
