Job summary

Elite Hospice is seeking a Social Worker - Hospice Per Diem to provide medical social services under physician direction and an interdisciplinary group. You will help clients and families understand social and emotional factors impacting health and coping strategies.

You will contribute to a daily, integrated Plan of Care and guide the care team in connecting patients with community resources. There is potential to serve as Bereavement Coordinator where needed.

Qualifications

  • Master's Degree in Social Work from an accredited program.
  • 1+ years of social work experience in a health care setting.
  • Current Driver's License and access to a vehicle or reliable transport.

Responsibilities

  • Assist the patient, family, physician, and care team to understand social and emotional factors affecting health.
  • Contribute to a comprehensive Plan of Care on a daily basis.
  • Instruct team members on available community resources as needed.
  • Possibly serve as Bereavement Coordinator and outline bereavement services within the plan of care.

As the Social Worker - Hospice Per Diem, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms. Primary Responsibilities

  • Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
  • Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
  • Instructs health care team members on community resources available to assist patients on a as needed basis
  • Able to function as Bereavement Coordinator and supervise the provision of bereavement services reflective of patient family if needed. Establishes a Plan of care that addresses bereavement needs with clear delineation of services to be provided

  • Master's Degree from a school of Social Work accredited by the Council of Social Work
  • 1+ years of social work experience in a health care setting
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
Preferred Qualification
  • Licensed Social Worker in the state of practice
  • 1+ years of social work experience in a health care setting
  • Current CPR certification or ability to complete within 90 days of hire
  • Bereavement Coordination experience
  • Experience with establishing a plan of care for bereavement needs
