Job summary

Adoration Health, an affiliate of BrightSpring Health Services, seeks a Home Health Social Worker in Carthage, TN. You will provide 1:1 visits to patients and families, assess needs, and help access community resources to improve social and health outcomes.

Qualifications include a Master’s Degree in Social Work, 5+ years of clinical social work, and an active LCSW or LMSW license. This PRN position offers flexible scheduling and strong team support.

Qualifications

• Master’s Degree in Social Work required.
• Five+ years of clinical social work experience.
• Current LCSW or LMSW license required.

Responsibilities

• Provide 1:1 visits to patients and families.
• Assist clients in obtaining government funds, education, or treatment.
• Secure information on medical, psychological, and social factors and evaluate them.
• Help clients with plans for meeting needs and mobilizing resources.
• Assist clients in modifying attitudes and behavior for better social functioning.
• Develop service plans to restore optimal social and health adjustment.
