Job summary

New York State Office of Mental Health's Central New York Psychiatric Center is recruiting for a Licensed Master Social Worker to provide professional social work services to individuals with mental illness within a multidisciplinary, recovery‑oriented environment.

As part of the Groveland Satellite Unit in Livingston County, you will perform crisis intervention, discharge planning, and develop and monitor individualized treatment plans, while coordinating with community providers for continuity

Qualifications

• Must be licensed and registered by NYS as LMSW.
• Candidates from outside or within State Service can apply.
• Transfer eligibility if you have one year of permanent competitive or 55-b/55-c service.

Responsibilities

• Providing crisis intervention, service linkages, and discharge planning.
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed.
• Determining discharge needs and appropriate community resources.
• Establishing and maintaining relationships with community providers to ensure continuity of care.
