Qualifications

Responsibilities

  • Interact professionally with patients of all ages from diverse backgrounds.
  • Complete biopsychosocial assessments and/or psychiatric evaluations.
  • Develop and maintain a patient-centered treatment plan.
  • Provide education and counseling to individuals and families.
  • Provide psychotherapy and crisis intervention as indicated.

  • Refer patients to community resources as indicated.

The Clinical Social Worker II is a licensed, experienced professional who is the clinical expert in the assessment and treatment of patients, families and social work consultation to the interdisciplinary teams in a variety of inpatient and ambulatory settings in the Yale New Haven Health System. The Clinical Social Worker II consistently performs beyond required job specifications, serves as a role model and mentors colleagues as evidenced by progression through the social work advancement program. As the clinical expert, the scope of practice includes, but is not limited to, diagnostic and teaching/mentoring. The clinical social worker will work collaboratively with members of the health care teams throughout the organization.

Responsibilities
  • 1.Interact professionally and with persons of all ages and from diverse cultural, socioeconomic, educational, racial and ethnic backgrounds, genders, sexual orientations, lifestyles, and physical abilities.

  • 3.Develop and maintain a treatment plan responsive to patient/family concerns, problems and clinical impressions.
  • 4.Use a patient and family centered approach to engage individuals and families in a plan of care that addresses their identified needs.
  • 5.Provide education and counseling to individuals and families.
  • 6.Utilize recognized best practices to provide individual, couples, family and/or group psychotherapy and/or crisis intervention as indicated.
  • 7.Consult and collaborate with external providers/agencies/organizations including but not limited to protective services, court systems, outpatient therapists, work/education settings and housing supports.
  • 8.Provide referrals to community-based services for individuals and families as indicated.
  • 9.Provide information, consultation and professional support to interdisciplinary team members and colleagues within the hospital and community.
  • 10.Facilitate and/or participate in interdisciplinary rounds and other patient related discussions to promote integrated care and improve patient outcomes.
  • 11.Initiate and maintain contact with managed care representatives to obtain pre-certification and ongoing authorization, to ensure reimbursement for services, as appropriate. Submit charges for services rendered as appropriate.
  • 12.Demonstrate initiative and leadership in developing services and programs to enhance patient and family care, including development of evidence-based practices.
  • 13.Contribute to individual, team, departmental and organizational quality assurance and performance improvement.
  • 14.Document timely, accurate and clinically relevant entries of social work activities in the electronic medical record.
  • 15.Navigate systems and provide advocacy for patients and families as indicated.
  • 16.Orient, precept, mentor and supervise students/trainees, junior colleagues and new employees.
  • 17.Participate in projects/ work groups and/or other duties as assigned by supervisor.

    • 18.1Maintain and enhance knowledge base and skill set through educational opportunities and meet expectations set forth by State licensure, Hospital and departmental standards.
Qualifications
EDUCATION

Master's Degree from an accredited school of social work.

EXPERIENCE

For Connecticut, consistent with LCSW licensure requirements, 3,000 hours of post-masters social work experience and 100 hours of post-masters clinical supervision.For Rhode Island, consistent with LICSW licensure requirements, 24 months of experience licensed as a licensed clinical social worker under appropriate supervision.Qualifying social work experience for advancement to this position from the LCSW I position includes one year of experience at Yale New Haven Health System and three years of experience as a licensed social work clinician or the equivalent.

LICENSURE

Licensed Clinical Social Worker (LCSW) from the State of Connecticut, or Licensed Independent Clinical Social Worker (LICSW) from the State of Rhode Island, or those holding licenses from other states, will need an 'endorsement' and meet the State of Connecticut or State of Rhode Island license requirements.

SPECIAL SKILLS

Ability to perform independently and provide clinical practice in a health care setting; to supervise social workers pending licensure and participate in teaching within the department, hospital, and university setting. Computer experience and knowledge of word processing, spreadsheet, and data entry systems.
