Job summary

Anchor Co-Living in California is seeking a compassionate Live-In Care Coordinator to support residents recovering from hospitalization and housing instability. You will be embedded in a residential home, coordinating care and services while fostering a stable, respectful living environment.

This role blends clinical knowledge, housing navigation, and daily resident support to move members from recovery toward independence, with emphasis on dignity and proactive communication.

Qualifications

• LVN or higher clinical credential required.
• Experience in care coordination, case management, or related services.
• Strong written and verbal communication skills.
• Experience using electronic medical record (EMR) systems.
• Excellent organization, documentation, and follow-through.
• Ability to reside on-site in an Anchor Co-Living home.
• Commitment to maintaining professional boundaries and confidentiality.

Responsibilities

• Coordinate medical appointments, transportation, medications follow-up, and access to services.
• Develop and maintain individualized care plans.
• Help residents identify housing opportunities and resources.
• Support housing applications and required documentation.
• Maintain documentation and care plans in EMR.
