Job summary

Kenosha Community Health Center – Pillar Health is seeking an experienced Family Practice Nurse Practitioner to provide high-quality, patient-centered care to patients across the lifespan in southeastern Wisconsin. The NP will independently assess, diagnose, and treat within their scope of practice while collaborating with an interdisciplinary care team.

The role requires a Wisconsin APRN license and FNP certification, with a minimum of 5 years in family or primary care, plus DEA and BLS

Qualifications

• Graduate of an accredited Nurse Practitioner Program.
• Current Wisconsin Advanced Practice Registered Nurse License.
• Current national board certification as a Family Nurse Practitioner or equivalent.
• Minimum of 5 years of clinical experience in family or primary care.
• Current DEA registration for prescribing controlled substances.
• Current BLS certification.

Responsibilities

• Provide comprehensive preventative, acute, and chronic care to patients across the lifespan, beginning at birth.
• Perform patient assessments, physical examinations, and medical history reviews.
• Diagnose and manage acute and chronic health conditions.
• Order, perform, and interpret diagnostic tests and imaging.
• Prescribe medications, treatments, immunizations, and other therapies as appropriate.
• Develop and implement individualized treatment and follow-up plans.
• Educate patients and families on medications, treatment options, and disease prevention.
• Maintain accurate documentation in the electronic health record.
• Collaborate with physicians and interdisciplinary care team members on complex patients.
• Participate in quality improvement efforts and uphold organizational standards.
