Job summary

McGregor PACE in Cleveland, OH is seeking a Life Enrichment Coordinator to plan and implement therapeutic recreational activity programs for participants. The role supports frail and elderly residents, including those with dementia, and works with an Activity Director to coordinate engaging activities.

You’ll report changes, assist with assessments, and help coordinate a multidisciplinary care plan while ensuring compliance with PACE regulations.

Qualifications

• High School Diploma required; Bachelor’s degree in art, music, or recreational therapy preferred.
• Certified Activity Coordinator preferred.
• Minimum two years of experience coordinating services for a frail or elderly population; dementia experience desirable.

Responsibilities

• Implements the activity program designed by the Activities Director for frail, older adults including those with dementia or physical impairments.
• Plan and implement individual and group activities that meet participant needs.
• Report participant status changes to the Activities Director and other staff as relevant.
• Assist with initial and periodic participant assessments as needed.
• Coordinate 24-hour care delivery plans with the multidisciplinary team and document assessments and care plans.
