Job summary

Regency Integrated Health Services is seeking an LVN Charge Nurse for Weslaco Nursing and Rehabilitation Center in Weslaco, TX 78596. This PRN position supports quality care and the well-being of residents through assessments and individual care plans.

The LVN will perform rounds, administer medications, document, and coordinate with physicians, while supervising aides and ensuring policy-driven care transitions.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
• Experience working in a skilled nursing facility is preferred.

Responsibilities

• During rounds observes, assesses, documents, and reports resident condition/changes.
• Assures implementation of care plans as directed.
• Administers medications as prescribed.
• Conducts rounds with physician and records visits in clinical records.
• Performs narcotic reconciliation with incoming and outgoing shifts.
• Oversees and monitors aides.
• Initiates and completes Incident and Accident Reports per facility policy.

Job Details: Job Location: Weslaco Nursing and Rehabilitation Center - Weslaco, TX 78596. Position Type: PRN. Job Category: Nurse. Primary Responsibilities: The LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.

Essential Functions

• During rounds observes, assesses, documents, and reports resident condition/changes
• Assures implementation of care plans as directed
• Administers medications as prescribed
• Conducts rounds with physician and records visits in clinical records
• Performs narcotic reconciliation with incoming and outgoing shifts
• Identifies significant changes in the condition of residents and takes necessary action per facility policies and procedures
• Oversees and monitors aides
• Initiates and completes Incident and Accident Reports per facility policy
• Discharges residents per facility policy
• Completes Nursing Assessment for Physician Calls prior to calling the physician on any change in resident condition
• Communicates with resident’s point of contact when they ask for a status update or if there is a change in condition
• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator
