Job summary

Skyline Healthcare Center-San Jose is seeking a License Vocational Nurse (LVN) to provide nursing care under physician orders in a team setting. The LVN will perform exams, administer medications, participate in care planning, and supervise fellow LPNs and nursing aides.

Qualifications include graduation from an accredited Nursing program, a valid state LPN/LVN license, and current CPR certification. This role emphasizes patient education and adherence to regulatory standards in a California

Qualifications

Graduate of an accredited school of Nursing.
Valid State licensure as an LPN/LVN.
Current CPR certification.

Responsibilities

Assesses patients by physical examination, including pertinent diagnostic testing to determine health status.
Administers medications and treatments.
Participates in the care planning process.
Supervises LPNs and nursing assistants.
Communicates with physicians regarding changes in resident’s conditions, diagnostic test results, etc.
Documents assessments and care in compliance with standards of care and company policy.
Educates patients and their families on health-related issues.
Completes required forms and documents in accordance with company policy and state and/or federal regulations.
