Job summary

Guardian Angel Home Care in San Jose, CA is seeking a Licensed Vocational Nurse (LVN) to provide private duty nursing care for a patient in the San Jose area.

This role features a consistent Monday–Friday schedule from 11:00 AM to 5:00 PM (30 hours per week), offering one-on-one nursing services in the patient’s home with competitive pay and a supportive clinical team. The LVN will document care electronically and collaborate with an RN and physician.

Qualifications

• Active California LVN license in good standing.
• Graduate of an accredited vocational nursing program.
• Current BLS/CPR certification.
• Strong communication and patient care skills.

Responsibilities

• Provide dedicated one-on-one skilled nursing care to a patient in their home.
• Monitor the patient's condition, vital signs, and overall health status.
• Administer medications and treatments as prescribed and within the LVN scope of practice.
• Assist with activities of daily living and patient care needs as appropriate.
• Observe and promptly report changes in the patient's condition to the supervising RN and physician.
• Maintain accurate and timely clinical documentation.
• Communicate professionally with the patient, family, and interdisciplinary care team.
• Follow the established plan of care and all applicable agency and state regulations.
• Maintain a safe, comfortable, and supportive environment for the patient.

Preferred Qualifications

• Previous private duty nursing or home health experience
