St. Joseph's Home Health, Inc. is seeking a dedicated Licensed Vocational Nurse (LVN) to join our compassionate healthcare team in Texas.

The LVN will support planning and delivery of patient care under the RN, maintain infection control, and ensure accurate documentation. The role emphasizes excellent communication, flexible scheduling, and a commitment to high-quality care for our patients and their families.

Qualifications

• Graduate of an accredited school of vocational nursing.
• Proof of current LVN license required.
• Proof of current CPR certification required.
• Ability to work a flexible schedule.
• Minimum 1 year of LVN experience in a clinical setting preferred.
• Excellent interpersonal and communication skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Identify patients' physical and environmental needs from documentation and team reports.
• Plan and implement care in conjunction with the RN.
• Provide care utilizing infection control measures for staff and patients.
• Assure continuity of quality patient care with proper documentation.
• Prepare regular clinical and progress notes and summaries.
• Assist patients with activities of daily living and teach self-care techniques.
• Provide and maintain a safe environment for the patient, including emotional support.
• Maintain client confidentiality at all times.
• Follow emergency procedures during catastrophic events or incidents.
