Job summary

St.-Joseph seeks an LVN to support nursing staff in a patient-facing role. You will assist with planning and implementing care, document clinical notes, and help patients with activities of daily living while teaching self-care techniques.

The position requires graduation from an accredited VN program, current LVN license and CPR certification, plus the ability to work a flexible schedule. A reliable transportation method is preferred.

Qualifications

• Graduate of an accredited vocational nursing program.
• Must hold an active LVN license and CPR certification.
• Ability to work a flexible schedule and adapt to staffing needs.

Responsibilities

• Under the direction of the RN, assist in identifying patient needs from records and evaluations.
• Participate in planning and implementing care with the RN.
• Provide care using infection control measures to protect staff and patient.
• Ensure continuity of quality care with proper documentation.
• Prepare regular clinical/progress notes and summaries.
• Assist patients with activities of daily living and teach self-care.
• Maintain a safe environment and protect patient confidentiality.
• Follow emergency procedures for catastrophic events or changes in patient condition.

Education

Responsibilities

• Under the direction of the RN, assist in identifying the physical and environmental needs as evidenced by documentation, clinical record, case conference, team report and evaluations
• Participate in planning and implementing care in conjunction with the RN
• Provide care utilizing infection control measures that protect both the staff and patient
• Assure continuity of quality patient care delivered with appropriate documentation
• Prepare regular clinical/progress notes and summaries
• Assist the patient with the activities of daily living while teaching appropriate self-care techniques
• Provide and maintain a safe environment for the patient, including an optimum physical and emotional environment
• Maintain client confidentiality
• Follow emergency procedures in the event of any catastrophic event, incident, e.g., accident, injury, or significant change in the patient's condition

Qualifications

• Graduate of an accredited school of vocational nursing
• Proof of current LVN license
• Proof of current CPR certification
• Ability to work flexible schedule
• Minimum 1 year experience as an LVN in a clinical setting preferred
• Excellent interpersonal skills
