Job summary

Merakey is seeking a Licensed Vocational Nurse (LVN) to provide professional nursing services in a residential setting for individuals with developmental disabilities and complex behavioral/medical needs. The role includes assessments, treatment, and coordination of care, with on-site medical support and on-call responsibilities.

Compensation is $35.00–$38.00 per hour, based on experience, with a focus on ensuring health and safety and collaborative work with the team in a residential

Qualifications

• LPN/LVN with CA license and valid driver's license.
• Minimum 1 year of experience with individuals with developmental disabilities and/or mental health issues.
• Residential environment experience preferred.

Responsibilities

• Implements program policy and procedures per regulations and guidelines; provides medical support including scheduling and follow-up of medical appointments.
• Collaborates with Program Director and Administrator to ensure health and safety of individuals.
• Provides on-site medical support to individuals in the program setting.
• Represents Merakey at external meetings and conferences as assigned.
• Maintains knowledge of program regulations and assists in licensure preparation as needed.
• Maintains regular contact with families and external stakeholders.
• Assists with on-call duties.
• Performs other duties as required.

POSITION SUMMARY

Perform professional nursing duties including assessments, treatment and follow through resolution of all health issues.

WORK LOCATION TYPE

Spends approximately 75% or more of time inside, providing care forindividuals withdevelopmental disabilities and complex behavioral/medical needs in a residential EBSH setting.

Compensation: $35.00-$38.00, based on experience

GENERAL DUTIES/RESPONSIBILITIES

• Implements program policy and procedure according to program regulations and guidelines
  Provides overall medical support to individuals who receive services to include scheduling of medical appointments and follow up as needed.
• Works closely with Program Director and Administrator to ensure health and safety of individuals receiving services
• Provides on-site medical support to individuals
• Represent Merakey at all external meetings, conferences, etc. as assigned
• Be knowledgeable and adhere to program regulations, assisting in preparation for annual licensure as applicable
• Maintain frequent contact with family members, as deemed appropriate
• Maintain frequent contact with external stakeholders as needed
• On-call responsibilities
• May perform other related duties when assigned or required
