Job summary

Family Health Centers of San Diego seeks a licensed therapist to join our outpatient team. You will provide counseling to assigned patients, complete timely documentation, and contribute to ongoing care plans within a supportive, multidisciplinary environment.

The role requires active California licensure (LMFT/LCSW/LPCC or psychologist) and collaboration with case managers, ensuring comprehensive care for diverse patient populations in a hybrid work setting.

Qualifications

• Active registration in good standing with the State of California as a licensed LMFT, LCSW, LPCC or Psychologist.
• CA licensure requirements compatible with outpatient mental health care.
• Ability to coordinate with a multi-disciplinary treatment team.

Responsibilities

• Provide counseling sessions to assigned patients.
• Complete all required documentation accurately and timely.
• Perform initial assessment and ongoing care.
• Participate in development of individualized care plans.
• Address co-occurring disorders.
• Work as a member of the multi-disciplinary treatment team.
• Coordinate with Case Managers and patient support staff.
• Connect patients to internal/external resources as appropriate.
• Perform other duties as assigned.

For more than 55 years, Family Health Centers of San Diego’s (FHCSD) mission has been to provide caring, affordable, high-quality health care and supportive services to everyone, with a special commitment to uninsured, low-income and medically underserved persons. Duties and Responsibilities

Requirements

Must have an active registration in good standing with the State of California as a Licensed Marriage and Family Therapist (LMFT), Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselors (LPCC) or Clinical Psychologist.

Work Environment

FHCSD's services are all outpatient and designed for those struggling with mild to moderate acuity levels. You will have support from other therapists, case managers, a scheduling team, administrators, and a wide range of other medical professionals in our facilities.
