Qualifications

• Active Illinois license required (LCPC, LCSW, or LMFT).
• Experience with adults and child/adolescent populations preferred.
• W2 fee-for-service productivity model.

Responsibilities

• Provide individual therapy to adults and possibly children/adolescents.
• Collaborate with multidisciplinary care teams to coordinate treatment.
• Maintain accurate clinical documentation in the EMR system.
• Deliver high-quality behavioral healthcare with a focus on patient outcomes.
