Licensed Social Workers at OMH are part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness. Duties may include, but are not limited to:

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care
