Job summary

LCMC Health in New Orleans is seeking a Licensed Master Social Worker to join our multidisciplinary care team. As a social worker, you will assess psychosocial needs, advocate for patients, provide counseling, and coordinate discharge planning resources for complex medical cases.

You will collaborate with clinicians, identify barriers to care, refer to community services, and offer crisis intervention when needed.

Qualifications

• Licensed Master Social Worker (LMSW) or eligible with LMSW within 6 months of hire.
• Experience in care management or hospital social work is preferred.
• Strong assessment and counseling skills with a patient-centered approach.

Responsibilities

• Conduct comprehensive psychosocial assessments and document findings.
• Identify barriers to care and coordinate interventions with the care team.
• Provide crisis intervention and supportive counseling as needed.
• Refer patients/families to community resources and assist with discharge planning.
• Collaborate with interdisciplinary teams to support care goals and patient rights.
