Meritus Health is seeking a Social Worker or Counselor at Brook Lane Main Campus in Hagerstown, MD. This full-time role requires licensure at the graduate level (LCSW-C, LCPC, LGPC, or LMSW) and a Master’s degree in counseling from an accredited program.

You will conduct psychosocial assessments, provide counseling, develop treatment plans, and connect patients and families with community resources, while guiding them through illness-related challenges with empathy and professionalism.

Qualifications

• Master’s degree in counseling from an accredited school.
• Maryland graduate level licensure (LCSW-C/LCPC/LMSW/LGPC) required.
• One year of healthcare experience preferred.

Responsabilités

• Conduct psychosocial and mental health assessments to identify patient needs.
• Provide counseling, support, and therapeutic interventions to patients and families.
• Develop and implement individualized treatment plans.
• Support patients and families as they adjust to illness and other challenges.
• Provide information, referrals, and connections to community resources.
