Qualifications

• Master's Degree in Social Work or Marriage and Family Therapy (LCSW, MSW, MFCC, or LPC).
• Current Social Work or Marriage and Family Therapist licensure/certification in the state(s) of practice.
• Minimum of one year of experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or other acute diagnosis.
• Current BLS (Basic Life Support) First Aid and CPR certifications.
• Demonstrates effective interpersonal, verbal, and written communication skills.
• Ability to work independently and as part of a team.
• Demonstrates strong organization skills and attention to detail.

Responsibilities

• Conduct comprehensive evaluations, assess the social, emotional and support needs of patients and their families, and set measurable objectives with the rehabilitation team.
• Identify and assist patients/families to utilize appropriate community resources to achieve objectives.
• Provide direct social work intervention and counsel patients and families in coping with social and emotional responses to injury/illness.
• Regularly reassess effectiveness of treatment plan and progress toward goals.
• Communicate patient needs and progress to the treatment team, physicians, patient and family members.
• Consult with medical providers regarding patient evaluation, progress and discharge.
• Observe/document changes in status and report to relevant coordinators and treatment team members.
• Maintain and submit evaluation, treatment and billing documentation per regulations.

Licensed Clinical Social Worker (LCSW) - Neuro Community-Based Care

Part-Time / PRN

We are seeking a Licensed Clinical Social Worker to support individuals and families navigating the emotional, behavioral, and life changes that come with neurological conditions.

This role focuses on real-world psychosocial support delivered in home and community environments, helping patients and families adapt, stabilize, and rebuild function in daily life.

You will be part of a neuro rehab team that values autonomy, flexibility, and deep clinical impact.

Responsibilities

• Conduct comprehensive evaluations, assess the social, emotional and support needs of patients and their families, and set measurable objectives and goals that are formulated in conjunction with the rehabilitation team. Adjust treatment plan to achieve maximum results in independence and/or function.
• Identify and assist the patient and/or family to utilize appropriate community resources to achieve measurable objectives. Utilize special skills and judgment to resolve adverse social and emotional responses connected with the injury/illness.
• Provide direct social work intervention and instruct and counsel the patients and family members in treating and coping with social and emotional responses connected with the injury/illness.
• Regularly re-assess effectiveness of treatment plan, progress of patient towards goals and the need for modifying goals or treatment.
• Communicate patient's needs and progress to the treatment team, physician, patient, family members, and others as applicable.
• Consult with medical providers regarding patient evaluation & treatment, progress of the patient & discharge from treatment.
• Observe/document and reports changes in the condition/status of the person receiving services to the Clinical Coordinator, Program Manager or DCM, attending physician and other treatment team members.
• Maintain and submit evaluation, treatment and billing documentation in the format and timelines required by the Company, facility, funding source, local, state and federal regulations.
• Adhere to standards of the Company's Code of Business Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.
• Perform other duties as assigned.
