You will develop social histories, create care plans, participate in interdisciplinary conferences, provide individual and group support, coordinate discharge planning, and help families adjust during transitions. We value reliability and compassionate care in a long‑term care setting.

Qualifications

• Bachelor’s degree in Social Work (BSW) or equivalent.
• Certification as Social Worker Designee or relevant education.
• Experience in long-term care settings preferred.
• Strong communication and interpersonal abilities.

Responsibilities
