• Minimum of one year of social work experience preferred
• Complies with all relevant professional standards of practice
• Current CPR if applicable
• TB questionnaire, PPD or chest x-ray if applicable
• Current Health certificate (per contract or state regulation)
• Must meet all federal, state and local requirements
• Must be at least 18 years of age

The Licensed Social Worker collaborates with themulti-disciplinary team to provide input in the development of the plan of carefor those clients requiring social work intervention. The Licensed Social Worker will providesupport, advocacy, and resources to individuals and families facing variouschallenges.
