Job summary

Live Well Homecare is seeking a licensed social worker to perform assessment, care planning, and case coordination for patients and families. You will act as a liaison between healthcare providers and the community, developing and evaluating social work plans and psychosocial assessments.

Responsibilities include coordinating services, advocating for patients, referring resources, and ensuring appropriate placement.

Qualifications

• Master’s degree in social work is required.
• Licensure as CSW/CST in Hawaii is required or equivalent.
• Basic Life Support certification is required.
