Job summary

FutureCare in Baltimore seeks a licensed Social Worker to coordinate psychosocial services for residents in our skilled nursing facilities. You will advocate for residents, provide crisis intervention, and support the care team to ensure high standards of compassionate care.

This role emphasizes discharge planning, family outreach, and collaboration within an interdisciplinary team to promote residents’ wellbeing and rights throughout their stay.

Qualifications

• Must have either a bachelor’s or master’s degree in Social Work.
• Experience in skilled nursing or sub-acute health care preferred.
• Current Maryland Social Work license required.

Responsibilities

• Coordinates medically related social services to improve residents’ daily functioning.
• Assists residents and families coping with illness and stay in the facility.
• Addresses advance directives and helps determine the medical decision maker.
• Provides individual, family, and group services to maximize psychosocial well‑being.
• Promotes residents’ rights and dignity within the care plan.
• Participates in developing an interdisciplinary plan of care.
• Provides support and grief counseling as indicated.
• Oversees discharge planning coordination to other settings.

• Assists in meeting the psychosocial needs of residents, their families, and significant others as necessary in coping with the effects of illness, disability, treatments and stay in the facility
• Addresses advance directives and assists in the determination of the appropriate medical decision maker
• Provides individual, family and group services designed to maximize psychosocial well-being
• Promotes actions by staff members that maintain or enhance each resident’s rights and dignity in full recognition of each resident’s individuality
• Assists in the development of an interdisciplinary plan of care for residents
• Provides support and grief counseling as indicated.
• Oversees and coordinates the process of discharge planning to all other settings

Qualifications

• Must have either a bachelor’s degree and/or master’s degree in Social Work
• Experience in a skilled nursing or sub-acute health care setting is preferred
• Must possess a current valid Maryland Social Work license
• The ability to function as a member of an interdisciplinary team
• Excellent interpersonal skills to appropriately relate to residents, their families, and community agency personnel
• Ability to prioritize and organize tasks to meet deadlines and proven ability handling distractions well
• Ability to navigate and document in an electronic health record systems
