Qualifications

Responsibilities

• Develop detailed social histories and assessments for residents and identify social/emotional needs.
• Create individualized care plans to support resident goals and well-being.

• Provide individual and group support to residents and families during crises or transitions.
• Assist with new resident orientation and help residents and families adjust to the facility.

Your Impact as a Social Worker

In this role, you will:
