Job summary

A national healthcare staffing firm is seeking a Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in Chicago, Illinois. This role involves coordinating care for participants, conducting assessments, and developing care plans. Candidates should have 2+ years of relevant experience, a valid driver's license, and be ready to work in a positive environment with competitive salary and benefits including medical, dental, and vision coverage.

Qualifications

• Must be Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW).
• 2+ years of professional experience as an LSW or LCSW.
• 2+ years of Case Management, Community Health, or Care Coordination experience.
• Must be able to travel locally (valid driver's license and auto insurance required).
