Job summary

Synergy Care is seeking a qualified Physical Therapist Assistant/Rehab Director for a full-time, on-site position at The Traditions – Clearfork in Fort Worth, TX. This role serves residents in an Independent/Assisted Living community and emphasizes patient-centered care in a supportive environment.

The PTA will work under a Physical Therapist, implement treatment plans, monitor progress, and collaborate with families and the care team to ensure quality outcomes and ongoing professional growth.

Qualifications

• Graduate of an accredited PTA program (or graduating within 6 months).
• Strong communication and interpersonal skills are required.
• New graduates encouraged to apply.

Responsibilities

• Provide physical therapy treatments under the direction of a Physical Therapist.
• Assist in implementing individualized treatment plans.
• Monitor and document patient progress.
• Communicate effectively with patients, families, and care team.
• Maintain accurate and timely documentation.
