Job summary

South Pacific Rehab Services is seeking a licensed Physical Therapist Assistant for a full-time role in SNF/Long-Term Care. You will provide therapy under the supervision of a licensed PT, aiding in patient evaluation and referrals, and collaborating with a multidisciplinary team to achieve optimal outcomes.

Responsibilities

Provide physical therapy treatments under the supervision of a licensed Physical Therapist, following state practice guidelines.
Assist in identifying appropriate patients for therapy and coordinate with the PT for evaluation and referral.
Collaborate with team to ensure quality outcomes.
Maintain documentation to meet regulatory requirements and obtain up-to-date physician orders.
