Coordinates care for patients and assists them in accessing services and resources that will help maintain them in the community in the least restrictive environment. **Job Duties*** Oversees all utilization management activity to include authorization of services as well as management of denial and appeal process.* Ensures all patient discharge planning documentation is clear, concise, timely, and congruent with treatment team recommendations.* Conducts clinical assessments for interpretation and treatment intervention with patient and families as directed by treatment plan.* Provides case management and care coordination services for clients/patients.* Intercedes on behalf of the individual patients to assist them in achieving and maintaining community stability. Acts as a liaison between patients, referral sources, and funders.* Manages all elements of mental health - voluntary and involuntary commitment processes as regulated by Pennsylvania Mental Health Procedures Act. (chapter 5100 regs)* Participates in quality improvement activities for the department and or program.**Minimum Qualifications*** Master’s Degree Social Work, Counseling, or related field.* Ability to remain patient and calm in stressful situations.* Must have knowledge of community resources and understanding of how to make appropriate referrals to community and other governmental agencies for services.* Understanding of the psychosocial/medical needs of clients in order to formulate an integrated, comprehensive treatment plan.* LCSW - Licensed Clinical Social Worker - State of Pennsylvania* LPC - Licensed Professional Counselor - State of Pennsylvania* LSW - Licensed Social Worker - State of Pennsylvania**Physical Demands** Lift and carry 40-50 lbs. Examples: Push/pull patients on bed, stretcher (requires 29 lbs. push force), lateral transfers up to 50 lbs. of the patient's weight. Frequent to continuous standing/walking. Patient transporters can walk 8-10 miles per shift. \*Patient care providers may be required to perform activities specific to their role including kneeling, bending, squatting and performing CPR.
