Job summary

Lehigh Valley Health Network is seeking a qualified mental health professional to provide assessment, diagnosis, treatment, and crisis intervention for adult and/or child patients with diverse mental health needs.

Responsibilities include coordinating care, facilitating access to services, and ensuring clear discharge planning documentation in collaboration with treatment teams and community resources.

Qualifications

• Master’s Degree in Social Work, Counseling, or related field.
• Ability to remain patient and calm in stressful situations.
• Knowledge of community resources and how to make appropriate referrals.

Responsibilities

• Oversees utilization management and authorization of services.
• Ensures discharge planning documentation is clear and timely.
• Conducts clinical assessments for interpretation and treatment with patients and families.
• Provides case management and care coordination.
• Intercedes to help patients achieve community stability.
• Liaises between patients, referral sources, and funders.
• Manages patient commitment processes as regulated by stateAct.
• Participates in department quality improvement activities.
