Key Responsibilities

• Conduct comprehensive initial assessments and ongoing evaluations to tailor treatment plans based on individual patient needs.
• Engage in regular communication with supervisors, healthcare professionals, and families to provide updates on client progress and effectiveness of therapeutic interventions.
• Develop a scheduled framework of appointments and home visits, ensuring thorough monitoring and support for each client, as necessary.
• Facilitate home visits to observe client environments, ensuring an understanding of context in treatment goals and needs.
• Collaborate effectively within interdisciplinary team meetings, contributing insights and strategies that enhance the overall treatment approach.
• Complete timely documentation including monthly and quarterly progress notes to maintain accurate client records and treatment histories.
• Organize and deliver training sessions to empower staff on the implementation of program plans and effective documentation practices.
• Conduct follow-up visits and interviews to provide continued support and monitor therapeutic progress.
• Serve as a liaison with the community, advocating for mental health resources and promoting awareness of available services.

Patient Population / Client Focus

Qualifications
