Job summary

MyCHN (Community Health Network) is seeking a Behavioral Health Therapist to provide comprehensive behavioral health services for patients of all ages within an integrated care setting.

You will conduct assessments, develop care plans, and coordinate with an interdisciplinary team to improve health outcomes while supporting wellness and patient education.

Qualifications

• Active and unrestricted Texas license as LCSW or LPC.
• Ability to practice independently and coordinate care.
• Eligibility for Medicare/Medicaid provider participation.
• At least one year of clinical experience in behavioral health.

Responsibilities

• Conduct behavioral health assessments and develop individualized care plans.
• Gather patient histories through interviews with patients and families.
• Provide individual, couples, family, group, and crisis counseling services.
• Support wellness management and coordinate referrals for specialized care.
• Collaborate with providers, psychiatrists, and interdisciplinary care teams.
• Educate patients on behavioral health conditions and health-promoting activities.
• Ensure timely and accurate documentation of all patient interactions.
• Manage follow-up care and ensure timely communication with patients.
• Support integrated care coordination across behavioral health, primary care, and community resources.
• Perform additional duties as assigned within scope.

What You’ll Do & Bring

What You Bring:

• Knowledge of public health principles and integrated care models


• Experience working with substance use disorders (SUD)


• Five years of clinical experience
