Job summary

Collage Home and Community Rehabilitation Partners in Austin, TX seeks an experienced Licensed Professional Counselor (LPC) to provide patient-centered care in clients' homes and communities on a PRN basis.

You will join interdisciplinary teams, develop individualized treatment plans, and work with patients with neurological injuries across the continuum from stabilization to rehabilitation.

We provide treatment for traumatic brain injuries, spinal cord injuries, CVAs, and other neuro or complex ortho diagnosis in the most functional setting there is - a person's own home and community.

We are looking for an experienced Licensed Professional Counselor (LPC) who wants to provide truly patient-centered care in the Austin, TX and surrounding areas - with PRN positions available for clinicians who will accept patients for the duration of their stay. Our interdisciplinary teams create integrated treatment plans that are practiced in patients' homes and communities. Our therapists are creative in their approach and incorporate a variety of activities to ignite interest while addressing functional goals. This evidence-based approach results in durable outcomes.

Qualifications:

• Completion of Master's degree in Psychology, Counseling or Social Work (LCSW) and state licensure required
• Excellent planning, organization, writing and time management skills are required
• Ability to communicate with families, professionals, and co-workers is a must
