Senior Care Therapy, LLC is seeking Licensed Professional Counselors to deliver in-person psychotherapy to the geriatric population across SNFs in NJ, NY, PA, and MD. The role includes full-time opportunities with no admin burden and a strong support system, including EMR, CEU reimbursement, and mentorship.

You will conduct individual and group sessions, develop treatment plans, and collaborate with facility staff to improve residents’ mood and functioning.

Qualifications

• Active and unrestricted Licensed Professional Counselor license in applicable state.
• Effective oral and written communication in English.
• Ability to provide in-person services at facilities, up to 45 minutes.
• Strong organizational skills and regulatory compliance awareness.

Responsabilités

• Conduct individual psychotherapy sessions in subacute or skilled nursing facilities.
• Assess, diagnose, and treat emotional and behavioral health conditions.
• Develop and implement evidence-based treatment plans for each patient.
• Collaborate with facility staff, interdisciplinary teams, and families.
• Maintain accurate clinical documentation in accordance with regulatory standards (EHR).
• Monitor patient progress and adjust treatment plans as needed.

We currently serve over 300 Skilled Nursing Facilities including sub-acute, long-term care, and assisted livings throughout NJ, NY, PA, and MD.

Key Responsibilities:

• Conduct individual and group psychotherapy sessions for patients in a subacute or skilled nursing facility.
• Assess, diagnose, and treat a wide range of emotional and behavioral health conditions, including depression, anxiety, adjustment disorders, and cognitive decline.
• Develop and implement evidence-based treatment plans tailored to the needs of each patient.
• Collaborate with facility staff, interdisciplinary teams, and family members to support patient care.
• Maintain accurate and timely clinical documentation in accordance with regulatory standards using our clinician-designed electronic medical/health system (EHR).
• Monitor patient progress and adjust treatment plans as needed.

Requirements:

• Active and unrestricted license to practice as a Licensed Professional Counselor in State applying for.
• Effective oral and written communication in English
• Basic proficiency with technology, including electronic health records (EHR).
• Strong organizational and documentation skills, with attention to regulatory compliance.
• Ability to provide in-person services at assigned facilities, up to 45 minutes.
• Ability to walk, stand, and move between patient rooms and offices within the facility throughout the day.
• Full Vaccination and Booster Status may be required in some facilities.

Preferred Qualifications:

• Previous clinical experience in a subacute, long-term care, or geriatric healthcare setting.
• History of treating Anxiety, Depression and Adjustment Disorders.

• Psych or Addictions experience is a plus.

• Excellent time management and organizational skills.
