Job summary

Nemours is seeking a LPN for Primary Care at the St. Francis location. The role supports a team-based primary care setting in Wilmington, DE, with a Monday–Friday schedule (8:30 or 9:00 start to 5:00–5:30 end). Bilingual English/Spanish is preferred.

Gathers patient data, administers meds and vaccines, assists clinicians, and performs point-of-care testing while delivering care appropriate to age and development. Active DE LPN license and current BLS are required.

Qualifications

• Graduate of accredited Licensed Practical Nurse program.
• Active DE LPN licensure required.
• Current BLS certification required from American Heart Association.

Responsibilities

• Gather patient data and collaborate with healthcare team on plan of care.
• Provide care that meets professional standards of practice and regulatory requirements.
• Safely administer medications and vaccines as prescribed by the provider.
• Assist provider with procedures within the LPN license scope.
• Perform point of care testing.
• Provide age- and developmentally appropriate care to primary and specialty care patients.
• Perform and document patient intake including height, weight, head circumference, blood pressure, vision and hearing screen, reason for visit, medication reconciliation.
• Participate in departmental and system-wide quality improvement activities.
• Screen patient and family calls, gather information, escalate and refer appropriately.

Hours would be Monday thru Friday from either 830-5 or 9-530.

Essential Functions:
