Job summary

Mohawk Valley Health System's Rehabilitation and Nursing Center is seeking a Licensed Practical Nurse (LPN) to provide general nursing care to residents and their families. You will support health needs and advocate for patient comfort in a caring, home-like environment.

LPNs collaborate with physicians, registered nurses and a multidisciplinary team, participate in case finding and health coaching, and may work days, nights, or weekends with flexible options for full-time, part-time or per-diem

Qualifications

• Graduate of an accredited school of licensed practical nursing program.
• BSN/MSN preferred.
• Current New York State licensure as an LPN.
• BLS certification required within 6 weeks of hire.

Responsibilities

• Provide general nursing care to residents.
• Collaborate with physicians, RNs and multidisciplinary care team.
• Support patient-centered care and health coaching.

Job Summary

At MVHS, a Licensed Practical Nurse (LPN) provides general nursing care to patients, families and caregivers at the Rehabilitation and Nursing Center (RNC).

LPNs play a vital role in providing compassionate care to our residents and their families. We’re offering many opportunities for various shifts including days, nights and weekends; we offer flexible scheduling options for full-time, part-time and per-diem positions.

The Licensed Practical Nurse assists and advocates for the patient and family’s health in support of the patient centered approach and delivery model of care, and is an active care team member. Responsible for collaborating with physicians/ providers, registered professional nurses and multidisciplinary care team members, a Licensed Practical Nurse will also perform tasks and responsibilities within the framework of case finding, health coaching, and supportive care under the direction of the registered professional nurse or licensed physician or licensed health care provider. Nursing shall be performed in accordance with the standards outlined for Licensed Practical Nurse in the NYS Education Law, NYS Office of the Professions, and by the State Nurse Practice Act. A Licensed Practical Nurse will follow the mission, vision, values, policies and procedures of Mohawk Valley Health System.

Education/Experience Requirements

Required:

• Graduate of an accredited school of licensed practical nursing program

Preferred:

• Bachelors or Masters of Science in Nursing from accredited nursing program

Licensure/Certification Requirements

Required:

• Current New York State licensure as a Licensed Practical Nurse
• Basic Life Support Certification - Must receive BLS certification within 6 weeks of hire date
