Job summary

Elara Caring is seeking a Licensed Practical Nurse Behavioral Health to join our home health team in Shelton, CT. You will follow individualized care plans, coordinate with the RN Case Manager, and educate patients and families on self-care techniques to improve health outcomes.

The role involves documenting medications, communicating with physicians and care teams, and delivering compassionate, high-quality care.

Qualifications

• 1 year of clinical care experience.
• Ability to frequently lift up to 50 pounds and travel between patient homes.

What is Required?

• Current State License as a Licensed Vocational / Practical Nurse


• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing


• 1 year of experience in a clinical care setting


• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment


• Must possess a valid driver’s license, maintain an acceptable driving record, OR have reliable transportation to travel between patient homes and assigned work locations.


• Experience in a hospice or home health environment is preferred

Benefit eligibility can vary by position and shift status.

The base salary for this position is $30.26 to $45.00 per hour, based on the company’s good faith estimate at the time of posting. Actual pay will be determined based on factors such as education, experience, skills, and internal equity.

This role may also be eligible for additional compensation, which could include performance-based bonuses, commissions, on-call pay, or other incentive earnings. These components are not guaranteed and depend on individual and/or team performance.
