Job summary

Ambercare Homecare is seeking a Licensed Practical Nurse to join our Albuquerque, NM team on a PRN/part-time basis. You will provide direct patient care under the direction of a registered nurse and implement the plan of care in a home health setting.

Responsibilities include accurate documentation, patient education on self-care, and coordination with other disciplines. Requirements: NM LPN/LVN license, CPR certification, and reliable transportation with valid license and insurance.

Qualifications

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience; home health or medical/surgical experience preferred.
• Currently licensed as LPN/LVN in the state.
• Possess and maintain CPR certification.
• Must have reliable transportation, valid driver’s license and auto insurance.

Responsibilities

• Provides direct patient care as defined by the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Documents accurately and timely according to the plan of care.
• Educates patient and family/caregivers on self-care per diagnosis and plan of care.
• Coordinates home health services and reports needs to the RN or Clinical Supervisor.
• Uses and/or prepares equipment and supplies adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
• Provides health care instructions to the patient as appropriate.

What You’ll Do:

• Provides direct patient care as defined in the state Nurse Practice Act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Assesses and provides patient and family/caregiver education regarding self-care techniques and information pertinent to diagnosis and plan of care.
• Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Uses and/or prepares equipment and supplies effectively and efficiently adhering to aseptic technique.
• Participates in personal and professional growth and development.
• Performs other duties as assigned by the registered nurse.
• Provides health care instructions to the patient as appropriate per assessment and plan of care.
