THRYVE Healthcare in Crestwood seeks a dedicated Licensed Practical Nurse (LPN) to provide total nursing care to residents on the assigned unit during PM shifts. You will supervise team members, develop individualized care plans, administer medications, and ensure adherence to facility standards to support high-quality resident care.

Qualifications

• Graduation from an accredited nursing education program.
• Active LPN license in Illinois.
• No prior abuse convictions.
• Strong communication and teamwork skills.
• Ability to follow policies and deliver empathetic care.
• Able to work with diverse resident populations.
• Physically able to perform nursing duties.

Responsibilities

• Provide total nursing care to residents on the assigned unit during the shift.
• Participate in developing and implementing individualized care plans with the healthcare team.
• Administer medications and treatments as prescribed; evaluate effectiveness.
• Maintain an environment promoting residents' physical, social, and psychological well-being.
• Educate residents and families on care plans and procedures.
• Monitor and support team member performance; assist with development and corrective action.
• Delegate and ensure completion of nursing tasks to team members.
• Stay current with facility policies and ongoing nursing education.
• Recognize changes in resident conditions and respond appropriately.
• Document care and responses per facility guidelines.
• Act as charge nurse: answer questions, resolve issues, support care decisions.
• Follow all facility safety policies.
