Job summary

Lehigh Valley Health Network (LVHN) Endocrinology at Cedar Crest is seeking a licensed practical nurse to join a collaborative care team. You will collect patient histories, perform triage, coordinate tests and referrals, and support providers during examinations and procedures.

You will educate patients, manage test results, schedule appointments, and help maintain patient safety and high-quality care in a fast-paced outpatient setting at our Cedar Crest location.

Qualifications

• Technical School Diploma LPN Program.
• Ability to manage violent situations or dangerous scenarios.
• Basic computer skills, critical thinking and concentration.
• Knowledge and competence reflecting current nursing practice.
• American Heart Association Basic Life Support in PA.

Responsibilities

• Collect patient history and reason for visit.
• Complete observation, physical exam, triage and data analysis.
• Track and log lab/diagnostic tests and present results to provider.
• Educate patients and ensure current, appropriate materials.
• Assist with prescription refills, scheduling and referrals.
• Assist providers during procedures and coordinate appointments.
• Intervene to prevent patient risks based on observed factors.

Summary

Works as part of a care team to improve the execution of quality requirements, increase throughput, improve efficiency, and enhance the overall patient experience. Provides care to designated patients and care coordination across the continuum. Performs administrative functions in the practice.

Job Duties

• Assists provider by collecting patient history and reason for visit.
• Completes observation, physical exam, telephone triage, and analysis of diagnostic data.
• Ensures that all ordered laboratory and diagnostic tests are tracked and logged and that the provider is presented with all test results.
• Provides education to patients, ensures education materials are current and appropriate, and evaluates the effectiveness of patient education.
• Assists patients with inquiries for prescription refills, appointment scheduling, and referral authorization.
• Assists providers during procedures and examinations and performs instrument and room clean‑up.
• Coordinates internal and external appointments for new and established patients based on patients' medical needs, preferences, and the practice schedule.
• Intervenes to prevent, minimize, or correct any actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors.
• Participates in telephone triage by obtaining clinically appropriate data and symptoms; advises and instructs patients according to established protocols and/or orders from the provider.

Minimum Qualifications

• Technical School Diploma LPN Program
• Ability to establish and maintain control of violent patients or dangerous situations.
• Basic computer skills, critical thinking, and ability to concentrate.
• Knowledge and competence that reflects current nursing practice.
• American Heart Association Basic Life Support - State of Pennsylvania

Physical Demands
