Schedule: Day Shift, M-F 8:00-4:30 alternating M-F 8:30-5:00 pm

Summary

Works as part of a care team to improve quality, increase throughput, enhance efficiency, and improve the patient experience. Provides care to designated patients and coordinates care across the continuum. Performs administrative functions in the practice setting.

Job Duties

• Assist providers by collecting patient history and reasons for visits, conducting observations, physical exams, telephone triage, and analyzing diagnostic data.
• Ensure all laboratory and diagnostic tests are tracked and logged, and provide providers with test results.
• Educate patients using current and appropriate educational materials; evaluate the effectiveness of education.
• Assist patients with inquiries regarding prescriptions, appointments, and referrals.
• Support providers during procedures, assist with instrument and room cleanup.
• Coordinate appointments based on medical needs and practice schedule.
• Intervene to prevent or correct patient risks based on observation of physiological, behavioral, and environmental factors.
• Participate in telephone triage, obtaining relevant data and advising patients per protocols and provider orders.

Minimum Qualifications

• Technical School Diploma from an LPN Program
• Ability to manage violent or dangerous situations
• Basic computer skills, critical thinking, and concentration abilities
• Knowledge reflecting current nursing practice
• American Heart Association Basic Life Support certification (upon hire)
• LPN license for Pennsylvania (upon hire)

Physical Demands

Ability to lift and carry 25 lbs., frequent sitting/standing, keyboard use; may include kneeling, bending, squatting, and performing CPR as required.
