Job summary

Sturdy Memorial Hospital is seeking an experienced Licensed Practical Nurse (LPN) for our ambulatory care team. The role provides direct nursing support under the direction of an RN or provider, including intake, triage, procedures, medication administration, and patient education.

You will maintain accurate documentation, ensure patient confidentiality, and collaborate with a multidisciplinary team to deliver high-quality care.

Qualifications

  • Graduate of an accredited LPN program.
  • Current MA LPN licensure.
  • Active BLS certification (AHA or ARC).
  • At least 1 year nursing experience in ambulatory/physician office.

Responsibilities

  • Provide direct nursing care within LPN scope under RN/physician supervision.

The Licensed Practical Nurse (LPN) provides direct clinical support to providers and patients in an ambulatory care setting, delivering nursing care within the scope of practice under the direction of a Registered Nurse and/or provider. The LPN supports patient care through assessment, intake, triage, clinical procedures, medication administration, patient education, care coordination, and accurate clinical documentation. This role collaborates with providers and interdisciplinary team members to promote high-quality, patient-centered care while ensuring compliance with regulatory requirements, professional nursing standards, and organizational policies.

Job Description
Education/Training:
  • Graduate of an accredited Licensed Practical Nurse program
Licenses/Certification:
  • Current Licensed Practical Nursing (LPN) licensure in Massachusetts
  • Active Basic Life Support (BLS) certification through American Heart Association (AHA) or American Red Cross
  • Responsible for keeping state licensure, continued education and BLS certification current.
Required Qualifications and Skills:
  • Minimum of one (1) year of nursing experience in a physician office, ambulatory care, outpatient, or comparable clinical setting.
  • Strong clinical nursing skills, knowledge of patient care practices, and medical terminology proficiency.
  • Ability to perform patient assessments, recognize changes in patient condition, and appropriately triage clinical concerns.
  • Ability to obtain, monitor, and accurately document vital signs and patient health information
  • Strong interpersonal, verbal, and written communication skills.
  • Ability to build rapport with patients, families, providers, and colleagues.
  • Ability to work collaboratively in a team-based healthcare environment
  • Strong organizational and time-management skills with the ability to manage multiple priorities in a fast-paced clinical setting.
  • Ability to maintain accurate clinical documentation and protect patient confidentiality.
  • Demonstrate commitment to patient experience, quality improvement, and continuous process improvement.
  • Ability to maintain clinical competency and comply with regulatory, professional, and organizational standards
Preferred Qualifications and Skills:
  • Experience providing patient education regarding treatment plans, medications, and preventive care.
  • Experience administering medications, injections, and immunizations in accordance with established protocols.
  • Experience performing telephone and/or in-person triage in a medical practice setting.
  • Experience using electronic medical record (EMR/EHR) systems and familiarity with physician office workflows.
  • Experience managing provider inboxes or patient messaging through an EMR.
  • Experience with prior authorizations, referrals, and care coordination.
  • Experience supporting staff training or onboarding activities.
  • Experience working in a multi-provider or specialty medical practice.
Essential Job Functions:
  • Provide direct nursing care and clinical support to patients in accordance with the Licensed Practical Nurse (LPN) scope of practice, organizational policies, and professional standards.
  • Perform patient intake activities, including obtaining and documenting vital signs, medical histories, assessments, and other clinical information to support safe and effective patient care.
  • Collaborate with providers and interdisciplinary team members to coordinate patient care, facilitate treatment plans, and ensure continuity of care.
  • Triage patient concerns, respond to patient inquiries, and appropriately escalates clinical concerns to providers or other healthcare team members.
  • Assist with examinations, procedures, diagnostic testing, and other clinical interventions while ensuring patient comfort, safety, and quality outcomes.
  • Administer medications, injections, immunizations, and treatments in accordance with established protocols, provider orders, and scope of practice requirements
  • Provide patient and family education regarding health conditions, medications, treatment plans, preventive care, and follow-up instructions to support optimal health outcomes.
  • Support care coordination activities, including referrals, prior authorizations, prescription refill processes, and patient follow-up efforts.
  • Maintain accurate, timely, and complete documentation within the electronic medical record (EMR) and other applicable systems while ensuring patient confidentiality.
  • Prepare, maintain, and stock clinical work areas and examination rooms to promote efficient workflow, infection prevention, and regulatory compliance.

  • Ability to assist with patient positioning, transfers, and mobility activities as required
  • Ability to visually assess patients, read medical records, and accurately document information.
  • Ability to communicate effectively with patients, families, providers, and other healthcare personnel in person and by telephone.
  • Ability to work in a fast-paced clinical environment requiring sustained concentration, attention to detail, and the ability to manage multiple priorities.
  • Ability to safely operate clinical and office equipment used in the performance of job duties.
  • May be exposed to infectious diseases, bloodborne pathogens, bodily fluids, and other occupational hazards common to healthcare environments; appropriate safety precautions and personal protective equipment are required

Salary Range: $25.16-$38.42

A successful candidate’s actual compensation will be determined after taking factors into consideration such as the candidate’s work history, experience, skill set, and education. This is not inclusive of the value Sturdy Health’s benefits package (if applicable), which includes among other benefits, healthcare/dental/vision and retirement. For annual salaries this is based on full-time employment.
