Job summary

VA Illiana Health Care System is seeking an LPN/LVN for outpatient clinics to deliver high-quality nursing care to adult and geriatric Veterans. You will work under RN/MD supervision and contribute to planning, implementing, and documenting patient care with emphasis on safety and comfort.

The role includes direct patient care, patient and family education, and coordination with the health care team in a fast-paced outpatient setting.

Qualifications

• Graduate of a school of practical nursing approved by the state agency or ACEN.
• Active, unrestricted LPN/LVN license in the United States or District of Columbia.
• At least one year of successful practice as an LPN/LVN or GS-4 equivalent.
• Evidence of transcripts or graduation from an approved program.

Responsibilities

• Provide direct patient care to adults in outpatient clinics.
• Coordinate care with RNs and physicians in the VA system.
• Educate patients and families on care plans, medications, and follow-up.
