Job summary

Hospice of the Panhandle in Kearneysville, WV is seeking a Licensed Practical Nurse (LPN) for night shift, 8 pm–8 am, on varying days with weekend rotation. This non-exempt, hourly role requires a WV eNLC LPN license and at least one year of nursing experience, preferably hospice or inpatient medical-surgical care.

The nurse will provide direct patient care, coordinate with the care team, educate families, administer medications, document care, and participate in ongoing training and staff

Qualifications

• WV eNLC Licensed Practical Nurse license required.
• Minimum of one year nursing experience, hospice or medical surgical nursing in an inpatient setting preferred.
• Must have own transportation and valid driver's license with current liability insurance.

Responsibilities

• Assess patients' physical, psychosocial, and safety needs and provide direct nursing care according to the plan of care.
• Collaborate with RNs and Nurse Managers to develop care plans.
• Educate patients and families on disease progression, symptom management, safety, and end-of-life care.

Located at the Hospice Inpatient Facility in Kearneysville, WV. Schedule is 8 pm - 8 am, on varying days, with weekend rotation.

General Statement of Duties:

The Licensed Practical Nurse (LPN) provides nursing care to patients and families based on assessed needs reflected in the plan of care.

Level of Responsibility:

Reports to Inpatient Facility Nurse Manager

Status:

Non-exempt, hourly

Pay Range: Starting at $30.85/hr.

Week night shift differential $2 (Monday-Friday, 8 pm - 8 am); weekend shift differential $3 (Saturday and Sunday, any times)

Duties and Responsibilities of Licensed Practical Nurse (LPN):

• Assess patients' physical, psychosocial, and safety needs and provide direct nursing care according to the plan of care.
• Collaborate with registered nurses and Nurse Managers to develop, revise, and maintain individualized care plans.
• Educate patients and families on disease progression, symptom management, safety, end-of-life care, and hospice philosophy.
• Administer medications, maintain medication inventory, verify medication accuracy, and report discrepancies.
• Document assessments, interventions, patient/family education, and other patient-related information in accordance with guidelines.
• Communicate effectively with physicians, patients, families, interdisciplinary team members, and external agencies to ensure continuity of care.
• Serve as a resource for pain and symptom management and provide updates to physicians as needed. Report suspected abuse or neglect in accordance with organizational policies and regulatory requirements.
• Participate in staff meetings, training, professional development, and orientation of students or new employees.
• Maintain hospice nursing competencies, comply with employee standards, and perform additional duties or assignments as needed.
• Obtains and maintains a current Jefferson County Food Handler's card.
• May function as hospice nurse in other settings as assigned.

Requirements

Education & Experience:

1. Ability to establish and maintain good working relationships with the patient and family, the attending physician, coworkers, and outside agencies.

2. Current West Virginia eNLC Licensed Practical Nurse license is required.

3. Minimum of one year nursing experience, hospice or medical surgical nursing in an inpatient setting preferred.

4. Able to provide own transportation, and a license to drive with current liability insurance. Driving record meets Hospice's insurance company standards.

Physical Qualifications:

1. Ability to transport self to office and patient care settings throughout the service area.

2. Ability to walk from car to patient's room in their care setting and perform essential job functions.

3. Ability to lift and move patients and equipment. Able to lift 50 pounds.

4. Ability to work 12-hour shifts; night shift is 8 pm - 8 am.

5. Ability to communicate verbally and in writing in English language.

6. Ability to hear normal conversation in person and via phone.

7. Ability to read normal or condensed print.

9. Ability to withstand odors.

Equipment Used:

Telephone, electronic equipment, office equipment, medical equipment and patient supplies used for patient care.
