Job summary

PruittHealth is seeking a Licensed Practical Nurse Case Mix Coordinator to join our team. You will support resident care planning and coordinate case-mix activities to ensure accurate documentation and appropriate care level determinations.

This role requires active LPN licensure and a proactive, collaborative approach in a long-term care environment. You will work with interdisciplinary teams to ensure compliant, quality patient care.
