Job summary

All American Assisted Living at Raynham is seeking an LPN to provide resident-centered nursing support in a compassionate, team-based environment. You will administer medications, monitor health, and coordinate services under nursing leadership to promote residents’ independence, dignity, and quality of life.

The role demands adherence to Massachusetts LPN scope, strong observation skills, and excellent documentation.

Qualifications

  • Must hold an active Massachusetts LPN license.
  • Knowledge of practical nursing standards and medication administration.
  • Strong observation and resident-monitoring skills.
  • Excellent documentation and communication with residents, families, providers, and pharmacies.
  • Ability to work within the LPN scope of practice and escalate when required.
  • Ability to supervise and provide guidance to resident-care staff.
  • Strong English written and verbal communication.

Responsibilities

What You'll Do
  • Administer medications and treatments according to provider orders, applicable nursing standards, and community procedures
  • Monitor residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, and daily functioning
  • Provide direct nursing care and resident support within the Massachusetts LPN scope of practice
  • Collect and document resident health information
  • Participate in resident assessments and service-plan reviews under the direction of the appropriate nursing leader
  • Participate in developing, implementing, evaluating, and updating resident care and service plans
  • Promptly report significant changes in resident condition
  • Initiate appropriate interventions within the LPN’s scope of practice
  • Notify the appropriate nursing leader, provider, family member, or emergency service when required
  • Receive, review, and carry out provider orders within the LPN scope of practice
  • Coordinate medication refills and communicate with pharmacies and providers
  • Document medications, treatments, observations, incidents, and nursing services accurately and promptly
  • Perform medication-cart, treatment-record, and documentation audits as assigned
  • Support resident admissions, transitions, hospital returns, and changes in level of care
  • Provide resident and family teaching within the LPN’s education, competency, and scope of practice
  • Promote wellness, hydration, nutrition, infection prevention, medication safety, and fall prevention
  • Respond promptly to resident emergencies and urgent needs
  • Participate in resident-care meetings and communicate relevant information to the Wellness team
  • Provide direction and supervision to resident-care team members as assigned
  • Delegate selected nursing activities only when permitted by Massachusetts law, nursing regulations, community policy, employee competency, and the resident’s condition
  • Provide the level of supervision and follow-up required for delegated activities
  • Assist with staff training, competency reviews, and required in-service education
  • Maintain resident privacy, dignity, rights, and confidentiality
  • Complete additional Wellness and nursing duties within the LPN’s lawful scope of practice
What We're Looking For
  • Current and valid Massachusetts LPN license
  • License must be active and in good standing
  • Knowledge of practical nursing standards, medication administration, and resident safety
  • Strong observation and resident-monitoring skills
  • Ability to recognize, document, and report changes in resident condition
  • Ability to work within the LPN scope of practice and seek RN or provider guidance when required
  • Strong documentation, organization, and time-management skills
  • Ability to communicate professionally and compassionately with residents, families, providers, pharmacies, and coworkers
  • Ability to communicate clearly and effectively in English, verbally and in writing, as required to understand provider orders, document nursing services, administer medications safely, report changes in condition, and respond during emergencies
  • Ability to provide appropriate direction and supervision to resident-care staff
  • Ability to work independently while recognizing when a concern must be escalated
  • Previous experience working with older adults is preferred
  • Assisted living, memory care, long-term care, home care, or similar nursing experience is preferred
  • Dependable attendance and punctuality
  • Ability to stand, walk, bend, reach, and remain active throughout the workday
  • Ability to safely lift, transfer, push, or pull up to (physical requirement) pounds with assistance or available equipment
  • Ability to successfully complete required background checks, health screenings, and employment requirements
Massachusetts Nursing Requirements
  • Must maintain an active Massachusetts LPN license throughout employment
  • All nursing services must be performed within the Massachusetts LPN scope of practice
  • The LPN may participate in resident assessments and care planning within the limits of the position, training, competency, and applicable nursing requirements
  • Responsibilities requiring independent RN assessment, nursing judgment, or another licensed provider must be referred or escalated appropriately
  • Delegation must be based on the resident’s condition, the nature of the activity, the employee’s qualifications and demonstrated competency, and the supervision available
  • The delegating or supervising nurse must provide appropriate guidance, monitoring, evaluation, and follow-up
  • Medication administration and nursing documentation must comply with applicable Massachusetts requirements and community procedures
