Job summary

Senior Lifestyle is seeking a Licensed Practical Nurse to provide licensed nursing services to residents in our community, adhering to federal and state regulations and company guidelines. Responsibilities include medication administration, admissions assessments, care planning, clinical tracking, and health-related consultations.

Must hold an LPN license current in the state and CPR certification; English proficiency is required for emergencies and communication.

Qualifications

• Training from an accredited licensed or vocational nursing program.
• General nursing focus.
• 1+ years experience in senior care or related field.
• Licensed Practical Nurse license current in the state of practice; CPR certification.
• Proficient English for job duties, supervisor communication and emergencies.
• Basic computer skills.

Responsibilities

• Medication administration for residents in line with Federal & State regulations and Senior Lifestyle guidelines.
• Admissions processing for new residents including assessments and documentation.
• Develop and maintain acute and long-term care or service plans.
• Track clinical data (e.g., wounds, infection control) as directed.
• Provide health-related consultations within the community.
• Notify physicians/family of health changes and document appropriately.
• Respond promptly to emergencies.
• Maintain communication with all departments and staff.
• Provide educational sessions for residents and staff.
• Understand role in Safety & Disaster Plan and maintain confidentiality.
• May discharge, assess and document clinically as needed.

Job Description

• Responsible for medication administration for the residents in compliance with Federal & State regulations and Senior Lifestyle Corporation guidelines.
• May be responsible for the admissions of new residents to include all admission assessments and documentation.
• Responsible for completing acute and long-term care plans or service plans.
• May be responsible for clinical system tracking (i.e. wounds or infection control) under the direction of the director.
• Provide general consultation regarding health-related concerns within the community.
• Notify physicians and/or family members of any change in resident’s health and provides proper documentation.
• Respond quickly to all emergencies.
• Maintain close communication with all departments and staff members.
• Provide educational meetings for residents and staff.
• Understand roll in the Safety & Disaster Plan.
• Maintain confidentiality of residents’ and prospective residents’ information.
• May be responsible for discharges, assessments, and clinical documentation as needed.

Qualifications

Level of Formal Education: Training from an accredited licenses or vocational nursing program

Area of Study: General nursing

Years of Experience: 1+ years

Type of Experience: Senior care or related field

Special Certifications: Must have a Licensed Practical Nurse’s License current in the state in which they are practicing. Also, needs to be CPR Certified

Language Skills: Acceptable proficiency in English to perform job duties, communicate with supervisor and other employees, and respond and communicate to emergency situations.
