Sixteenth Street Community Health Centers in Milwaukee seeks a Licensed Practical Nurse (LPN) for our school clinic. The role focuses on delivering patient-centered outpatient care in a busy clinic and supporting preventative health services.

The LPN will administer vaccines and meds under orders, help with CLIA testing, EMR documentation, and care coordination. Bilingual Spanish/English preferred; commitment to serving diverse communities is essential.

Qualifications

  • Graduate of accredited school for Licensed Practical Nursing.
  • Licensed Practical Nurse with current license and 2 years of experience in a clinical setting.
  • Current basic life support (BLS) certification or certification within 6 months through in-house training.
  • Ability to relate well to people from diverse ethnic and cultural backgrounds.
  • Bilingual (Spanish/English) preferred.

  • Assist in maintenance of EMR patient documentation: review patient data; assist nurse with follow-up.
  • Administer medications under orders, including vaccines and IV therapy as needed; assist triage nurse with screening calls.

JOB RESPONSIBILITIES:
  • Render excellent customer service and enhance the patient/clinical team relationship.
  • Assist in smooth flow of clinic, including but not limited to:
    • Greet and establish clinical relationship.
    • Prepare for clinic by reviewing charts for patient needs.
    • Provide translation for non-bilingual staff.
    • Set-up and stock exam room.
    • Prepare patients for medical visits by taking histories, BP, HT, WT, temperature, Respiratory Rate, Pulse, and anticipatory screenings based on the chief complaint, as needed (vision and hearing screening, EKG, NST, CLIA testing).
    • Prepare anticipatory referrals.
    • Manage flow of exam rooms.
    • Work to maintain a manageable schedule by noting openings and anticipating complex patients who may require additional MA support.
  • Perform CLIA testing, to include but not limited to:
    • Routine CLIA lab procedures as qualified.
    • Collect specimens and prepare for analysis.
    • Complete related documentation.
    • Alert provider of abnormal results as per policy.
    • Maintain cleanliness of lab area, restock lab as necessary.
  • Assist in maintenance of EMR patient documentation:
    • Review patient data on chart each visit.
    • Assist nurse with follow-up or project activities as assigned.
  • Assist patient with exiting material.
  • Provide patient centered nursing care in an outpatient clinic according to established scope of practice guidelines and standards of care.
  • Administer medications, under physician/provider orders, including immunizations, antibiotics, pain medications, vitamins, topical agents, Synagis, and insulin. Initiates, maintains, and discontinues IV therapy: administering selected fluids.
  • Assist the triage nurse with screening calls.
  • Serve as a resource for Medical Assistants.
  • Provide basic patient education on the performance of tasks, skills, treatment, and self-care.
  • Refer questions to registered nurses and physicians/providers per scope of practice guidelines and relay information back to patient as directed.
  • Participate and maintain projects as assigned. May include but not limited to:
    • Become CPR instructor certified and teach SSCHC classes.
    • Assist RNs with assigned projects.
    • Assist Medical Assistant Supervisor with compliance and safety procedures.
