Palmer Ranch Center for Rehab and Healing in Sarasota, FL is seeking licensed practical nurses (LPNs) to join our clinical team providing high-quality care in a resident-focused environment.

The role requires Florida AHCA background screening; we offer a supportive team environment and comprehensive benefits including health, dental & vision insurance, PTO, 401(k), flexible scheduling, and more.

Qualifications

• LPN license required in state of Florida.
• CPR certification required.
• Two years nursing experience preferred.

Responsibilities

• Admissions, discharge and transfers of residents.
• Medication requisitions with pharmacy and physicians.
• Medication administration.
• Follow infection control, emergencies and center policies.
• Collaborate with interdisciplinary team for best outcomes.
• Identify and report changes in residents’ condition.
• Supervise care in accordance with regulations and policies.
• Deliver nursing care for long-term or rehabilitative patients.
• Collect patient/resident data and report pertinent information.
• Implement and evaluate plan of care per state regulations.
• Direct and supervise care by other nursing staff when needed.
• Maintain documentation requirements and equipment knowledge.

Some of your Licensed Practical Nurse (LPN) Job Responsibilities include:

• Admissions, Discharge and Transfers of residents
• Medication requisitions with pharmacy and physicians
• Medication administration
• Following infection control, emergencies, and other center policies
• Collaborating with resident’s interdisciplinary team to provide the best outcomes
• Identifying and reporting all abnormal changes in residents’ condition to proper personnel per center policy
• Works under direct supervision in accordance with the state-specific Nurse Practice Act, facility Policies and Procedures, and nursing judgment.
• Delivers nursing care to patients/residents requiring long-term or rehabilitative care.
• Collects patient/resident data, makes observations, and reports pertinent information related to the care of the patient/resident.
• According to state-specific regulations, implements the patient/resident plan of care and evaluates the patient/resident response.
• In accordance with state-specific regulations, directs and supervises care given by other nursing personnel in selected situations.
• Maintains knowledge of necessary documentation requirements.
• Maintains knowledge of equipment set-up, maintenance and use, i.e., monitors, infusion devices, drain devices, etc.
• Maintains confidentiality and patient/resident rights, regarding all patient/resident/ personnel information.

Licensed Practical Nurse (LPN) QUALIFICATIONS:

• LPN (Licensed Practical Nurse) licensed by the State or Multistate License
• CPR Certification required
• Two Years prior nursing experience preferred
