Job summary

Signature HealthCARE in Roanoke Rapids, NC is seeking a dedicated LPN/LVN to provide high‑quality nursing care on 12‑hour shifts. This role supports residents through medication administration, wound care, ADL assistance, and adherence to care plans, while supervising nursing assistants.

You will work within an interdisciplinary team, maintain compliant documentation, and participate in QA activities. PRN opportunities and a people‑centered culture accompany comprehensive benefits and growth.

Qualifications

• Active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or Red Cross).
• Minimum one year of clinical nursing experience; long-term care preferred.
• Experience in a supervisory or charge nurse role is preferred.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.

Responsibilities

• Provide direct nursing care to residents, including medication administration and wound care.
• Assist residents with activities of daily living, ensuring comfort, dignity, and safety.
• Supervise nursing assistants and medication aides during shifts per policy.
• Monitor residents’ health status and report changes to the physician.
• Develop and review individualized care plans with the interdisciplinary team.
• Document accurately and timely per facility policy.
• Coordinate admissions, transfers, and discharges.
• Participate in QA activities, staff education, and surveys as needed.
• Perform other duties as assigned by the Director of Nursing.

How You Will Make a Difference

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.

What You Need To Make a Difference

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one (1) year of clinical nursing experience; long-term care experience, preferred.
• Experience in a supervisory or charge nurse role, preferred.
• Strong clinical observation, medication administration, and documentation skills.
• Knowledge of Federal and State long-term care regulations.
• Effective verbal and written communication skills.
• Ability to work independently, prioritize tasks, and exercise sound clinical judgment.
• Experience in a supervisory or charge nurse role.
• Familiarity with electronic health records (EHR) systems.
• Proficiency with Microsoft Office applications.
